client care overview
Our concierge care model affords each and every client a highly personalized and flexible care plan designed to meet the evolving needs of older adults.
Care is provided on an hourly, daily or temporary respite basis and can include support with basic activities of daily living and also more advanced care requiring the experience and skills needed to support a transition home from the hospital or rehab facility, or specialized conditions such as stroke or Alzheimer’s disease.
At Home Care Assistance of West Texas, we don’t just match you to a caregiver. You work with a full care team including a Client Care Manager who works closely with your family to make sure all your needs are met and the highest standard of care is maintained throughout the duration of service with Home Care Assistance.
